Interested students are required to submit an application which will include the following:
- A 500 word or less personal statement describing how you meet one, two, or all of the following eligibility criteria: Underrepresented in Medicine (UiM), significant economic need, and/or home medical school does not have a urology department.

Applications for the 2022 cycle are closed. Please check back in 2023 for information regarding the 2023 application process.
Students receiving a scholarship will be required to apply as a visiting student through the UW School of Medicine and Public Health and pay the $125 nonrefundable fee. This fee will be reimbursed to the scholarship recipient.
The Medical Student Education Office in the UW Department of Urology will work with the recipient to identify a four-week period for the elective that fits the availability of the program.
Scholarship checks will be issued upon arrival of the recipient to UW.
