To apply to the program, a candidate should forward a copy of his/her CV, a personal statement, and two letters of reference, including a letter of support from the program director recommending him/her for a fellowship and attesting to successful completion of the residency program.
Admission is contingent on the completion of an ACGME accredited urologic training program. Candidates are expected to take the Qualifying (Part 1) Examination of the American Board of Urology at the conclusion of residency training or be board certified by the ABU.

Options for interviews include the AUA meeting or applicants are encouraged to visit the medical center in Madison. Early June is the online deadline for the submission of rank and preference letters, with the match being completed in mid-June. Match results are announced shortly thereafter.
